At my last two practice sessions I have noticed that my Colt 1991 has started failing to feed the last round in the magazine. The ammo is mostly 230gr RNL reloads and once Fiocchi 230gr FMJ. I use Chip McCormick magazines and have a Wilson buffer installed. I bought the pistol used, so I have no idea as to the age/round count on the recoil spring.
The problem has not occurred with my duty ammo, only with my reloads and once with the Fiocchi. Any suggestions?

Most last round failures can be traced to weak magazine springs, or the combination of that coupled with an extractor out of adjustment.

Might want to check both.

How many magazines are you using? And does it happen with all of them?

I use a total of 7 magazines, 1 in the pistol and 6 on my belt. The failures have only happened with 2 of the Chip McCormick magazines (so far) both of which are fairly new.

I would say that there's your answer, the 2 mags. Possibly weak springs, or the feed lips are a bit out of adjustment..........as long as there's no hang up with the follower.

I agree with Wichaka. If you have a problem with only two of the mags all of the time, that's the problem. You can buy new mag springs for Chip mags and replace them easily. Also, I would throw away the shock buffer; you don't need it.
